It has been noticed that client’s attendance adversely affects his grades and when he comes to school late he falls behind in class which aggravates him. Client does a better job when he starts his day on a good note. Client appeared to be in an joyful mood, as evidenced by his positive approach toward peers and authority when greeting them at the start of class. Client displayed positive body language and a calm tone of voice. Client expressed appropriate thoughts about being in school, stating “I am happy to be here”. Client was supported for being on task and cooperative while teacher administered activity. Client was observed participating in his academics without needing assistance. Client was actively engaged in class discussion. Client made insightful comments which contributed to discussion. Client was observed participating in individual task. Client was able to appropriately ask for help when he didn’t understand directions. Client was able to verbalize positive feelings and emotions in school. Client was given positive feedback about his efforts and contributions. Client was respectful to his peers. Client was engaged and attentive to his individual responsibilities. Client utilized active listening skills when others were talking. Client is witnessed enjoying demonstrations that are presented. Client exhibits the appropriate attitude that is encouraged and inspired by staff and mental health professional. Client exhibits appropriate social behaviors while associating with others. Client complies to authority directives without complaining. Client appears to be learning and enjoying his day. Client joined his group for a counseling break. Client shares his daily experiences at school in order to reflect and debrief during session. Client has implemented interventions and strategies taught by QMHP in his daily activities. Client shows great attitude and high level of participation with his peers. Client complies to instruction and is seen working as a team. Client communicates well in group setting, expressing positive thoughts about activity. Client works well with others and seem to be elevating daily with his personal goals.
